By submitting this waiver, I, for myself, my heirs, and assigns, do hereby indemnify, defend and hold David Belanger, Aaron Thompson, and “Filipino Martial Arts of Texas”, and each and all subsidiaries, direct and indirect, and its and their instructors, assistants, associates and students, harmless from and against any cost, liability, claim, action or cause of action resulting from or arising out of any injury to myself or my property, or that I may cause to others or their property, whether now or in the future which occurs by virtue of my participation in the class as taught by David Belanger and/or Aaron Thompson in the Martial Art of Arnis De Malaya.
I hereby understand that I will be instructed in the Martial Arts, which should be used for self-defense only. I understand that I am fully responsible for the use of this art outside of my regularly scheduled classes, and that misconduct could lead to expulsion.
Furthermore, my participation in this class is conditional to standards, which have been set forth by David Belanger and “Filipino Martial Arts of Texas”, and my enrollment in classes may be revoked or suspended at the discretion of the instructor or assistants.
I have read and understood the document “Courtesy and Respect in the Classroom” prior to taking classes, and agree to abide by the guidelines set therein. I understand that failure to follow the rules may lead to my expulsion.
I acknowledge that I have read and that I understand this Indemnification Agreement, that I am at least 18 years old and that I am signing this Agreement of my own free will.
